Terms and Conditions Declaration Form for the
Loan of Film and Media Equipment
By signing this form, you are confirming that you have:

and agreed with your tutor all aspects of the production and particularly have
drawn his/her attention to any situations in which people or equipment may be
put at risk.

all participants of such matters and taken all reasonable precautions to secure
their safety.

all necessary clearance for recording and detailed all locations on your
booking form.
The Declaration is as follows:
I confirm that I
have discussed and agreed with my tutor all aspects of the activity, and have
drawn particular attention to any situation in which people or equipment may be
put at risk. I also confirm that I have informed my crew and all participants
in the activity of the above matters, and that I have taken all reasonable
precautions to ensure the safety of those participating in the activity. I
confirm that I have obtained all necessary clearances for the recording of my
project, that all locations are detailed on loan form and that I have discussed
with my Staff Supervisor any likely problems. I confirm that during my project
I will work in a professional and responsible manner.
Lost, damaged and overdue items and fees
During term time
resources are in constant use. Scheduling of equipment and facilities is
critical, and the success of the loan system relies on the timely return of
borrowed items. Failure to return borrowed equipment will deny other Students
or Staff expecting booked facilities to be available. If, for any reason, you
are delayed in returning equipment or loan items by the due date, please
contact the Film and Media Equipment Store Technician Eva Voutsaki (evoutsaki@studygroup.com)
as soon as possible. It may be possible to extend your booking, or make
alternative arrangements for equipment to be made available to the next user.
Under the terms of
the loan agreement, the signatory for the loan is responsible for all equipment
borrowed and must ensure its safe return to the Store for serviceability
checks and maintenance prior to re-issue. Equipment must not be sub-let by the
borrower, or directly passed to another. Passing equipment directly to another
student or colleague will not be seen as a proper handover of responsibility
and the signatory for the loan will remain fully responsible for proper and
timely return of the equipment.
All
items borrowed must be returned in the same state as issued and to the Film and
Media Equipment Store only.
Items of equipment must be returned to the equipment store and to Eva so that they are restocked through SISO. Returning equipment to the reception, or anywhere else in Friston, is unacceptable. Items which are retuned damaged or which are irretrievably lost will have to be replaced by the Store which will require the signatory for the loan to contribute towards the cost of repair or replacement. Students are strongly advised to take out insurance against such possibilities. Any equipment that goes missing or gets damaged will result to your fees deposit being withheld to pay off.

damaged must be accompanied with a written report to the Store.
Items
lost or stolen
Any loss or theft
of equipment must be reported immediately to the Store. The signatory of the
loan agreement will be required to provide a written report, providing full
details of the incident. The signatory of the loan agreement should report any
theft of equipment to the Police in the area where the theft is thought to have
occurred. Any incident involving the loss or theft of Sussex ISC property will
be investigated.
Late
returns
The department
does not operate a fee penalty system, and instead works on building trust with
the students. We trust that you will collect and return the equipment on the dates
and times you stated when making the booking. If you are late in returning
items you have borrowed, you will be seriously impacting on other student's
ability to access the resources. We operate a 3-strike system. After 3 strikes
your account may be suspended for a period of time suitable to the severity of
the late returns, evaluating how late the returns are, and the frequency of the
late returns. You will be required to meet with the Head of Film and Media in
order to re-activate your account.
Warning
Always be careful
when using Sussex ISC loaned equipment. Never leave equipment unattended when
in use and always store equipment away safely and securely at other times. Take
care to avoid damaging equipment, particularly with food and drink, extreme weather
conditions and detrimental environments.
